Wolds Community Wellbeing Team (CWT) currently have an exciting opportunity for qualified, enthusiastic and motivated social worker to join the team; we are currently recruiting to the post of Hub Social Worker. Please note you must hold a social work qualification and be registered/or applying to be registered with Social Work England to apply for this role.
Vale of York Clinical Commissioning Group funds the hub social work post, with a one year rolling funding agreement. This post works exclusively with Pockington GP practice as part of a multidisciplinary team (MDT) to provide support with preventing admission to and promoting the discharge from hospital for an identified group of patients. The post holder will also provide social work support to the MDT and patients admitted to the dedicated Hub beds based at Woldhaven Residential care facility. The overall aim of the service is to support and enable patients to maintain their independence and through strengths based social work enable them to remain living in their own homes. You will be required to work closely with health colleagues to provide a service which is dedicated to supporting and enabling people to maximise their independence
East Riding of Yorkshire Council (ERYC) Adult Services will employ you under the management of Wolds CWT. ERYC offers an excellent training package and provides clear development opportunities; We pride ourselves on providing a high quality induction for new staff. In addition, a fully supported ASYE programme is established within the authority for newly qualified social workers.
Wolds CWT is a busy adult social care team based in Driffield; this post has the use of offices in Pocklington, but is administratively based in Driffield. Agile working is actively encouraged within the authority and will be essential to fulfil the duties and responsibilities of this role. This is a vital post offering opportunity for personal and professional development. In order to ensure that the council discharges its statutory duties and responsibilities it is essential this post is filled by a registered social worker who has knowledge of working in an adult social care setting with experience of complex case work, multiagency working and assessment or someone is willing and eager to learn.
Social Workers are responsible for undertaking assessments for social care and determining eligibility in line with The Care Act 2014. Experience of undertaking Mental Capacity Act 2005 assessments and best interest decision making in accordance with the legislation is desired but full training will be given to the successful applicant.
Currently the teams operate Monday- Friday but this may be extended to cover a 7 day service in the future.
The ability to travel across a wide geographical area is necessary for which a mileage allowance is payable.
