An exciting opportunity has arisen to appoint a suitably qualified Senior Social Worker to be part of the Enhanced Hospital Team. You will be part of the in reach Hospital Discharge team supporting Scarborough and Bridlington Hospitals. There is scope for Agile and home working, with a good degree of flexibility in line with the needs of the service.
There is an expectation that this role will be to cover a 7 day service and that some weekend working will be a consideration for anyone applying for this role.
This is a fantastic opportunity for registered social workers to take a proactive role in supporting expanding and developing their skills; and for suitably qualified Social Worker to move into a Senior Role and furthering their professional career within a well-established local authority.
The role of senior social worker will be to work as an autonomous professional, demonstrating expert and effective practice in complex situations. This will include working with people and carers through the assessment process to identify, build and maintain family and social networks, to support and enable people and carers to actively stay well and independent, in line with strength based approaches a key in promotion of independence, wellbeing and choice. There is an expectation that you will have creative conversations with the people using services which focus on goal setting and outcomes. You must be a registered social worker with experience of adult social care settings. You must demonstrate a good knowledge of the Care Act 2014 and ability to work within the legislative framework of the Mental Capacity Act 2005, Deprivation of Liberty Safeguards and the Human Rights Act. Knowledge of safeguarding vulnerable adults and working within safeguarding procedures is key to this role.
As a Senior Social Worker there will be expectation of a high level of allocation and evaluation of the discharge process from the Acute Hospital Trusts in York, Scarborough and Bridlington.
You will be responsible for providing good quality accurate information, which will be shared with system partners to support the flow of discharges from the Hospitals.
You will work closely with the Team Manager and continue to develop a clear vision of what it needed to promote excellent team work. The current drive for strength based Social work is at the core values of the Enhanced Hospital Team and as a senior social worker you will be required to promote this within the team.
You must have excellent communication skills, the resilience to work under pressure and be flexible in your approach. You will be required to undertake the formal supervision of unqualified staff and support those entering the AYSE programme.
