An exciting opportunity has arisen to appoint a Care Coordinator to the busy Review and Peripatetic Social Work Team based at County Hall, Beverley for a fixed term period ending 31.07.2022. The team undertakes annual reviews, predominantly in residential homes and covers the whole of the East Riding geographical area. The ability to meet the travel requirements of the post is essential for which an allowance can be claimed.
The successful applicant will work in accordance with the Care Act 2014 demonstrating experience of working in Adult Services undertaking assessment for services and reviews. This will include a multi-agency approach working with people and carers to ensure that the care provided is person centred and meeting the person’s needs and working with collaboratively with partners and agencies to safeguard vulnerable people. An understanding of policy, operational practice and procedures relating to adults, safeguarding and carers is desirable.
You will work within the legislative framework relating to adult social care and will have some knowledge of Safeguarding Adults at Risk, Mental Capacity Act 2005, Deprivation of Liberty Safeguards and an awareness of care and support assessments.
You must have excellent communication skills, the resilience to work under pressure and be flexible in your approach.
You will also possess good IT skills and be expected to input data on the adult information database which is essential in collating data for the performance agenda as well as compiling a complete individual electronic social care record.
All new staff will receive a comprehensive induction, regular supervision, employee development reviews and training opportunities. The team currently operates between Monday and Friday however a move to seven day working is under consideration.
