An exciting opportunity has arisen for a full time Enabling and Transitions Officer on a temporary contract.

Futures+ is a specialist team that will manage the assessment and support for people with learning disability and/or autism across the East Riding and as we develop into a specialist service, we are further developing this role to better reflect the needs of the people we provide services for.

The role of the Enabling and Transitions Officer is to manage customers with Learning Disabilities, Autistic Spectrum Disorder and associated challenging behaviours.

The post holder will be responsible in supporting the social workers in the team to manage these cases that require intensive ongoing social work support and case management to enable them to function in their community.

The successful candidate should ideally have experience in a community well-being team setting. They will need to have a good understanding of the needs of clients with a learning disability, Autism Spectrum Disorder and associated challenging behaviours. It is essential that the post holder understands the needs of working in a multi-agency arena and is able to support in the management of challenging situations.

It is essential that the successful candidate has excellent communication skills, can work to own initiative and be person centred in their approach to their case load.
