The East Riding of Yorkshire Council’s Adult Social Care Service has a great opportunity to work with a dynamic team in the delivering of care and support to vulnerable adults. We need a suitably qualified, energetic and resilient individual to take up this exciting opportunity and help us to achieve our aims and offer a quality service to adult with social care needs.
We are seeking to recruit to a full time Care Coordinator post based in the Haltemprice Community Wellbeing Team on a permanent basis.
You will be responsible in the delivery of care to vulnerable adults, providing assessments under The Care Act and undertaking reviews of these assessments in the community and other settings which may include hospital and residential settings. You will support adults to maintain and promote their independence using a strength-based approach, this will include drawing upon skills, local knowledge, and wider community networks.
The role will require you to work in a range of situations and support clients at times of crisis. This will include participating in the role of duty and providing same day responses to crisis intervention. The role is demanding and pressured.
You must demonstrate some knowledge of the key legislations of Care Act 2014 Mental Capacity Act 2005, the Human Rights Act and knowledge of safeguarding vulnerable adults and working within safeguarding procedures is key to this role.
Excellent communication skills, an ability to manage your time effectively, working under pressure and being flexible in your approach are essential requirements of the role. You will also possess excellent written and IT skills and be expected to input data on the adult information database which is essential in collating data for the performance agenda as well as compiling a complete individual electronic social care record. The ability to use Microsoft applications is an advantage.
All new staff will receive a comprehensive induction, regular supervision and training opportunities. The team currently operates between Monday and Friday however a move to seven-day working is under consideration.
You will work in the Haltemprice area but may be expected to cover other areas therefore you will require access to transport and have a clean driving licence.
The team can facilitate a blended working approach (home and office).
