We are looking for a passionate, creative Team Manager/ Senior Social Worker who has proven leadership skills to join us as we look to remodel our hospital social work teams. This role demands a level of reliance along with a robust nature as we work closely with system partners to achieve safe and timely discharges from Hospital. We have been working with our colleagues in health and care right across the Humber and North Yorkshire system to explore new ways of working and to build a new model of care that supports a ‘home first’ ethos. Our overarching goal is that decisions regarding long term care, where possible, should not be made while a person remains on an acute hospital ward. This means that we are having to challenge some historic practice and test and try out new ways of working. We are building strong and lasting relationships with all our system partners and would love you to join us on this journey. You will need to be a natural change agent who is comfortable working with ambiguity but wants the opportunity to be bold and brave!! You will be joining established teams who are proactive and excited about the journey ahead. We are working closely with the Community Support Services and Occupational Therapy practitioners to strengthen our development under the Discharge to Assess model of Hospital Discharge. You will have the opportunity to lead ‘cross provider’ projects and inform and advise on our new model of practice and bring forward change. Ideally you will have evidenced experience of managing Social Work Teams and a style of leadership that incorporates a coaching and mentoring ethos to the Senior Social Workers across the Hospital Social Workforce.
