Specialist skills and knowledge Criteria
Essential
  • Qualified Occupational Therapist and/or good understanding of therapy assessment, particularly Skills for Daily Living or:
  • Qualified Physiotherapist with good knowledge of the role of physiotherapy and O.T. in re-ablement, particularly Skills for Daily Living or:
  • Qualified Social Workers with good knowledge of the role of physiotherapy and O.T. in re-ablement , particularly Skills for Daily Living.

Good knowledge of Assistive Technology to support Skills for Daily Living, Mobility, Transfers, Medication Management, Falls etc.

 

Good understanding of the ‘Strength Based Approach’ to assessment and support planning.

 

Experience of working in a hospital environment and/or good knowledge of Schedule 3 of the Care Act 2014 and the Discharge to Assess process.

 

Experience of working with Acute hospitals relating to Pathways 1,2 and 3 for patients.

 

Good knowledge of the Care Act (2014),in particular, Carers and the role of Advocacy.

 

Good knowledge of the Mental Capacity Act (2005) and the implications for individuals being discharged from hospital.

 

Good knowledge and experience of working with primary care, acute and community care trusts.

Job outline

Overall purpose

Provide leadership and management of staff and budgets for all Access and Prevention services within Adult Social Care. Develop and commission adult social care and related services with the ICS and NHS Trust  and other key partners,  working with the directorate management team and  Director, Commissioning and Market Development. Provide leadership to support effective change through strong collaboration with all key stakeholders.

Principal accountabilities

Responsible for all of the following for Access and Prevention services within Adult Social Care:

  1. Contribute to the development and implementation of Council and Adult Social Care and Health strategies.
  2. Develop and implement Council-wide strategies for health and social care for vulnerable adults, their families and carers.
  3. Advise Elected Members, The Executive Director of ASC & Health on all relevant issues relating to the services.
  4. Ensure information on services is collected and analysed in order to commission services or support to meet gaps in care.
  5. Advise Elected Members, The Executive Director of ASC & Health on all relevant strategic, operational and commissioning issues relating to the services.
  6. Lead service planning, development and implementation of change to ensure services are responsive to the needs of vulnerable adults, their families and carers and they meet the statutory duties of the Council.
  7. Prepare for and participate in inspections across Adult Services, providers’ services for which we have safeguarding responsibility, commissioned services and those delivered in partnership and elsewhere in the Council.
  8. Act as the ‘Nominated Individual’ to supervise and manage the in-house regulated activity:
  9. Ensure the services are complaint with service regulations and standards.
  10. Ensure the services are run well and safely.
  11. Ensure the services are managed and have the right resources.
  12. Play a leading role in the development and maintenance of strategic partnerships across the Council, ICS, private and voluntary sector and ensure the involvement of service users and the public in relation to service development and monitoring.
  13. Responsible for setting, monitoring, controlling budgets and authorisation of expenditure against defined budgets.
  14. Be accountable for designing, setting and achieving the relevant service objectives, standards, performance measures and activity targets set out in the Service Plans together with Council objectives and targets along with ensuring the delivery of joint health and social care targets.
  15. Foster, develop and maintain partnerships council-wide, cross agency and multi sector, including service users, carers and communities to ensure the best health and well-being outcomes are achieved.
  16. Work with national and regional bodies such as ADASS, the LGA, NHS England and CQC to ensure the Council is sighted on the national agenda and responds accordingly.
  17. Support the identification of best practice and adoption across services.
  18. Ensure the Executive Director and the Directorate Management Team have access to up to date and relevant information and advice in relation to Access and Prevention Services to support strategic planning.
